2024-2025 New Student Registration

Step 1 - Complete the Online Pre-Registration Form

* This form alerts us that you are new to town and collects your contact information, anticipated school start date and any additional information you wish to share.

Step 2 - Complete the Online Enrollment
* Register your child: eSchool Online Enrollment portal  (link) 
* eSchool Online Account Setup Instructions for creating an account for Online Enrollment  
* Creation of an account will add your child to our district-wide database and be where you upload the required documents for enrollment. You can also save your work and log back in to upload the required documents later.                      

ALL required documents identified below must be submitted to complete registration. Your child(ren) will not be enrolled in Wayland Public Schools until copies of all required documents have been received, reviewed and processed.

• 1. Student Birth Certificate (Copy of original - must have seal) or Adoption Certificate

• 2. Current Student Physical and Immunization Records and Health History (link) 
  

• Wayland Public Schools, in compliance with regulations of the Massachusetts Department of Public Health, requires the following documents before a student enters school: A copy of a physical exam that contains documentation of a vision screening, TB risk assessment, and immunization record.
  
  The school nurse must receive these documents as follows:
   * One year prior or 30 days after entry to the Wayland Public Schools
   * Entering grades Preschool, K, 4, 7, and 10
   * Annually prior to participation in school sponsored athletics

• 3. Proof of Wayland Residency: 
  

  Please Provide 1 of the below 5 options:
  Current Mortgage Payment and/or Property Tax Bill	Fully Signed and Executed Lease and/or Rental Agreement  (Must be executed by both parties)	Landlord / Owner of Property Affidavit (Must be Notarized by Wayland Town Clerk)	Fully Signed and Executed Purchase and Sale (P&S) Agreement  (Occupancy date must be before student can be registered)	Section 8 Agreement

   4. Proof of Wayland Occupancy:

• Please Provide 1 of the below 3 options:
  Gas / Oil Bill, Electric Bill, Home Telephone Bill (not cellular), Cable Bill or Water Bill  (Note: Bill must be dated within the past 45 days and parent / guardian name and address must be stated)	If no utilities, provide recent bill dated within the past 45 days showing Wayland residency address and name  (Note: A Residency Statement / Affidavit is required with this option)	Massachusetts Car Registration or Excise Tax Bill

  
  5. Proof of Identification (parent/guardian):

• Please Provide 1 of the below 4 options:
  Valid MA Driver's License	Valid MA Photo ID Card	Valid Passport / Green Card	Other Government Issued Photo ID

ALL the personal and residency required documents must be uploaded or submitted to complete registration and enrollment. Your child(ren) will not be enrolled in Wayland Public Schools until copies of all required documents have been received, reviewed and processed.

• 6. Complete Student Authorization for Release of Records (link) which allows your previous district to release school-related records, including Transcripts, State Tests, Individual Education Plan (IEP), 504 Plan, and Health Records.
  
  7. Provide Additional Required Documents (If Applicable):

  * Caregiver Affidavit
